GUIDELINES ON ONLINE ENROLLMENT FOR INCOMING GRADE 7 under Special Program in the Arts

Prerequisite for Enrollment:

1. The learner should have a general average of 83 and above, and no grade lower than 80 in any subject.

Special Guidelines for SPA Enrollees:

*Please comply to the following special guidelines BEFORE proceeding to the online enrolment.

1. Aspiring SPA learners shall attach the following outputs to the e-mail address of the teacher in-charge by specialization/major. Attachments MUST NOT EXCEED 25mb but quality of video/photo should not also be sacrificed. Submission of outputs runs on May 4 to 16, 2020.

1. Please also do include these information in your e-mail:

a. Complete Name

b. Facebook account

c. Active contact number of parents and the learner

2. QUALIFIED learners shall proceed with the online oral interview together with their parents. They will receive a confirmation via facebook messenger coming from the MAPEH Department Head, Mrs. Maribel S. Diolazo.

3. Meanwhile, administration of qualifying exam will no longer be conducted.

4. Learners who qualified both in the performance and interview may proceed to online enrolment. (insert link here)

General Guidelines for Enrolment:

1. An administrator e-mail account shall be created, and that will be made accessible by the persons involved in the Enrolment Procedure Team.

2. The Google Form shall reflect the text written in the enrolment form prescribed by the DepEd. A link and a QR Code shall be made accessible to the enrollees.

3. ONLY THE NEW, TRANSFEREE, BALIK ARAL AND REPEATER ENROLLEES FOR GRADE 7 ARE ALLOWED TO ACCESS THE LINK OR SCAN THE GIVEN QR CODE. (Concerns for other grade levels have a separate guideline to follow.)

4. Presentation of credentials shall be a case of school to school transaction.

List of credentials may include but not limited to:

· SF10

· General Average (in case SF 10 is temporarily unavailable)

· Certificate of Good Moral

5. The enrolment of the learner shall only be accepted upon submission of the enrolment form via Google Form AND compliance of the previous school attended to the required credential/s.

6. Once enrolment is accepted, the learner shall be classified as TEMPORARILY ENROLLED until needed documents/credentials are submitted.

7. Enrollees whose elementary school failed to comply with the submission of the required credential/s shall be labeled with PENDING REQUEST. This means that their enrolment is in a conditional status. (In this case, sectioning may arise as a problem.)

8. Other requirements such as photocopy of birth certificate, 2x2 picture and health record shall be presented on the opening of classes.

9. Sectioning of classes shall be accomplished by the class advisers.
